The following documents must be provided for enrollment: Birth certificate; Social Security card; blue Certificate of Immunization (IMM-50), which should include two measles immunizations and one for chicken pox; two proofs of residence; copy of parent’s driver license; custody papers where applicable. Registration will be permitted without immunization certificate, but certificate must be presented before school begins on Aug. 12. A child’s fifth birthday must be on or before Sept. 1 to enroll in Kindergarten. Child’s sixth birthday must be on or before Sept. 1 to enroll in first grade.
